it looks like your problem is Dreamweaver config and not PHP/Mysql/Apache. if you've started Apache, ftp the same code to the DocumentRoot outside of DreamWeaver and see if your code runs that way...
I don't use Dreamweaver for php, but my thought is that you may hvae hit the F12(Preview) and that only opens a browser pointing at your C:\ drive...where PHP can not run...i don't know how to set up a test server in DW, but its probably in the Define Sites Menu. When i tried to preview it again a message popped up saying "to preview pages containing server-side code, you need a testing server. Would you like to specify one now?" What does all this mean.
