Don't use the users file name at all.  

Create a table, use AutoInc, give the files references to the AutoInc.
The table contains info about what the document is about - a column for
committee number, a column for date, a column for ID of person
uploading, plus anything else that you need.  

Don't put the files in the database, but refer to them by details found
in the database.

If the person who uploads the file has their ID in the database for that
