Your script seems to rely on register_globals being on.  This is risky.  Use
the super global ($_POST) to check to see if your upload button has been
clicked.

To get to your specific issue, move_uploaded_file() makes a copy of the file
and places it in the destination and then deletes the original.  You should use
it to copy the file to the first destination and then use the copy() function
to make a copy of the file in the first destination and place the copy in the
second location.
