This is phpFormGenerator isn't it.
Your first mistake was downloading it, as protection is virtually non-existant
(unless it's been recently updated).
The p has a carraige return after it:
pt_register('POST','ZIP');
p
t_register('POST','Phone');
Also, you have a missing quote further down the code.
Spammers will love you for installing this :-)
