Just to clarify what I meant, I am not necessarily saying that all of this stuff needs to be driven out of the php-syslog-ng database. Perhaps you just use the PHP front-end to drive changes down to the syslog-ng.conf, or like you said, you create this second destination which is a FIFO and you have some perl script reading from the FIFO, and applying the rulesets that were created through the php-syslog-ng interface and saved in the database.

You will likely be running a periodic query on the database and processing that output. But in either case, what you are after is some kind of alerter service. At present, php-syslog-ng is nothing more than a log-presentation service. The alerter stuff can be added on but would be entirely separate from the front end.
