Hi,
I'm new on this ML and PHP. But if I can help ...
It seems you forgot a $ in your 'description' request in the '$sql' query.
$sql = "INSERT INTO tbl1 (name,location,length,size,description) VALUES
('$name','$location','$length','$size','description')";
