I agree with Pete completely...the best way to do this is on the client 
side with Javascript (or even a Java or Flash app). Search the internet 
for Javascript clock, Java clock or Flash clock and I'll bet you find a 
lot of results of clock apps you can download.

One side note...this could be done with PHP although it is very 
impractical and, hence, not worth doing this way. With PHP, you could 
just code the page to automatically refresh every so often (i.e. every 
second or every minute). Then, when the page refreshes, PHP would 
display the current time. Refreshing every second is just plain 
ridiculous...don't do it. Even refreshing every minute is too often and 
the visitor will wind up getting annoyed and leave.
