One thing I see is that you need to put the following code snippet
into the opening HTML <FORM> tag on the page where you are wanting
to populate the fields from the database. There's a full version of
how to do this just below the code snippet.
Try this...

Or, this...
(This form reads the data from the database, displays it in the form
fields, and then when any of the data is changed and the submit
button is clicked, the new or updated information is added to the
