If all programlistings in manual have two blank lines in the start and
at the end, they could be removed by some stylesheet tweaking.

The decision is up to the leaders of PHP documentation group. I'm just
saying that current practise is not in the line with "good DocBook
practise" (ask Norm Walsh if you don't believe me). But it's the thruth
that some stylesheet modifications could remove problems in output. So
without modifying sources we can get to the place where current medicine
is -- remove consequences not the cause of problem ;-)
