This is known, and PDF isn't created anymore until it's solved. From the very beginning of Chapter 46 to the end of all the appendices is truncated on the acrobat formatted files of your 4.1.2 php manual. I verified this problem via all the us mirros and the UK mirrors and found the same problems exist.
