This sounds good.  After everyone ponders this let's make
a decision and do it that way for all new parameters and
parameter changes.  Maybe we can make a role for these, 
much like we plan to do with seealso.  This would be easier
to parse a list of all new/changed parameters and make 
them consistent.

It is a <note>, but is a note with a <title>.  But yes, 
we want to come up with a better name for new parameter
and changed parameter titles.  Any suggestions? :)  This
can affect the new role proposed above.
