Sorry, guys. It was really interesting to work in PHPDOC team. Frankly speaking, I've got quite addicted. =) But two days ago my computer's power supply got damaged together with motherboard and probably HDD too. Now I'm at the front of the test computer of my former work in antivirus company answering emails about my offline status. I believe, that in three weeks or more the situation will be better. I do not like unfinished work, but for now just can't do anything except looking for local employment.
As for current XSL status, you can try to add to htmlhelp.xsl next string to include old customizations directly. There will be some errors probably, but there are also chances that manual will be successfully compiled. I can't test this.

<titleabbrev> or shortened title is used in navigation headers mostly, so it doesn't need to be rendered explicitly. Add
<xsl:template match="titleabbrev" />

No. It is my fault. While debugging HTML HELP project files I've forgot to
move out parameter
<xsl:param name="htmlhelp.only" select="1"/>
which caused only ToC, index and HHP files to be generated. Commited fix
to CVS.
