Due to using + instead of * in a regex, empty entities ('') are not
matched properly in mkindex.php. Case in point: phpdoc/en/contributors.ent
&frontpage.editors; Results in an E_NOTICE in bookinfo. Eg:
http://livedocs.phpdoc.info/index.php?l=en&q=bookinfo
Patch is available here:

Patch also adds code to prevent the E_NOTICE from occurring and instead
puts a HTML comment.
