[PHP-DOC] Another typo in appendices/migration5.xml

Hi!

The line 441 in appendices/migration5.xml -- "database" should be changed to 
"database server".
Otherwise the description of "ibase_server_info" is the same as of 
"ibase_db_info" (line 377).
