We used to have all lowercase urls, despite what the xml:id was.
This might be worth some discussion but I think we should go back to
having all lowercased urls.
I think it is important that we fix the markup rather then breaking
all possible standards more. Besides, it is easier to fix the markup
than it would be to hack this into PhD (not to mention it would make
PhD much slower).
So... are there any objections to lowercasing all xml:ids?
It will obviously break some URLs again though, but hopefully for the
last time..
This is probably for the best but lately I've been in a scared "avoid
outdating translations at all costs" mode. In other words, let's fix
the ids once and for all.
