So you were saying that with a stock install it works, but not your normal one. Could you check if the phpgw_accounts_cantrememberafter index last number is the same as the last id used in phpgw_accounts ? That could be a reason. Also, do you have errors in the webserver log ? The page I was talking about is the page where you have the list of users and where you can click on "add user".
