Problem identified and solved. There was an alias for 'manual' pointing to Apache manuals in httpd.conf. This appears to be a standard feature of Apache.

Commenting it out solved the problem, but I lost online access to the Apache manual.

I wonder if it would be wise to change the name in phpgroupware from 'manual' to 'manuals' to prevent a conflict.
