I'd highly recommend Dr. James Evans at Thomas Jefferson Univ. Hospital in Philadelphia, which isn't too far a drive for you. You may have seen him recently as he did an endoscopic resection live on the internet. While he is relatively young by doctor standards, he has already done this type of procedure hundereds of times. He also is young enough that he still spends plenty of time ednucating the patient both before and after the procedure.
