Bart ran into this -- the directory we create to cache the installed state
of a package is assumed to be consistent with reality if it exists, but we
create it empty on doing a refresh if it doesn't exist, leading pkg list to
say that no packages are installed.
This changes refresh to always create and populate the directory if it
doesn't exist, or is empty.
