I have reworked the webrev,
http://cr.opensolaris.org/~padraig/ips-7049-v2/, so that it now fixes
the problem for Joanie.
The problem was I was being too aggressive in not setting the accessible
descriptions during startup. I had expected that they would be set up
later but Michal's work in speeding up startup meant that the accessible
descriptions were not set up.

object after the package icon was removed.
