The following webrev contains fixes for the following issue:
13740 pkg state update should discard all entries not known or
installed

In short, if you uninstalled a package that was marked as renamed or
obsolete, and that package was no longer available from the publisher it
was installed from, it would still be listed in the output of pkg list
-afv and attempts to install it would still work (assuming the manifest
was still cached).
However, the above would only hold True until the next pkg refresh was
performed.
