The following webrev contains fixes for the following issue:
14181 catalog lazy-loader skips pkg.renamed actions

When the initial rename support was added to the system, this change got
missed. This issue could have prevented renamed packages installed from
a v0 repository from being seen as renamed.
A unit test has been added.
