Folks,
This is a small code review for a bug that Shawn found on Friday
evening. This can occur if a transport operation is interrupted by a
signal, in this case ^C, while a cleanup is in process.

Can I get one more reviewer for this change? It's small, about 25
lines.

So what of the remove_handle() call in cleanup_requests?

It also wasn't clear why the call to reset() had to be nixed in shutdown(), although it seems intentional.

Cheers,
