When TBDialOut transfers a number to a URI, it always removes the
leading '+' symbol and tries to replace it with the contents of the
configuration setting extensions.tbdialout.plus

By default, extensions.tbdialout.plus is empty so it simply removes the
plus.

For a valid tel URI, the plus should be left in place.

As a workaround, it is possible to edit the preferences and where it
says "Replace leading + with:", put a '+' symbol.
