apparently, there's no way to leave pacmd w/o killing the pulseaudio
server.
that's rather ridiculous since the idea of pacmd is obviously to
configure a server -- once it it is killed, all settings done in pacmd
are gone again.

it's easy to end up in the pacmd shell by mistake, so there should be
anon-destructive way back.

Either Control+C or Control+D will terminate pacmd without exiting the
server. The exit command means exit the pulseaudio server, which I
agree is confusing, but exiting pacmd is still possible. This is
documented in the manpage.
