This may be a bug in pulseaudio or in pavucontrol. Please post the output of pactl list sinks, to determine if this pulseaudio sees the 2 ports. Also, could you try pulseaudio and pavucontrol from testing (it will update a bunch of stuff unfortunately), to check if this is fixed by newer versions? Perhaps you can use a live-cd to avoid messing with your system.
