I have had this issue several times now. It happened in a KVM VM (Jessie) with seriously slow disk I/O (broken ZFS in the host _and_ misbalanced btrfs filesystem in the guest). Since I've fixed the ZFS and rebalanced the filesystem it did not happen again. Bernhard
