I'm unable to reproduce this problem. I just tested on debian/unstable in my qemu test installation, using
debootstrap sid /tmp/new http://ftp.se.debian.org/debian It completed successfully. This was with the debian kernel 2.6.17-1-686 and sysvinit 2.86.ds1-15. Could your problem be related to your self-made kernel? As far as I know, the sysvinit package have not changed its handling of /dev/initctl for a very long time.
