This is a bit odd of a focus issue, but I reproduce it consistently.

I run xfwm4 outside of the rest of xfce, under gnome-session.  I don't
expect that part is relevant.  I have Focus follows mouse set.  It mostly
works as expected.  However in one scenario I consistenly trigger, it
does not in fact follow the mouse:

Have at least two workspaces, with at least two windows each.  For
simplicity, pretend they each cover half the desktop, one on the left
and one on the right.  With the mouse over the left window, press the
key combination for the other desktop.  While the desktop is refreshing,
move the mouse over the right window.  Once the desktop is done
refreshing, the focus will be on the left window.  This can be addressed
in the immediate sense by moving the cursor off and back on to the right
window.

I think I've also seen the focus adjusted when a popup dialog appears
and disappears.

This really crimps my desktop style, and so far, other than that, xfwm4
has been a very welcome change from the last few WMs I've used.
