For my 4.2 instance, the destination folder was C:\Plone42\eggs\products.cmfplone-4.2.0.1-py2.6.egg\Products\CMFPlone. The Extensions directory was not present as a subfolder. After this, I restarted my Zope instance and finish migrating successfully.
