We have to add it to every record, e.g. the anchor names are unique
for each "part" of a page. That will result in some duplicated data,
but I don't think it will add that much to the total size of a
document. Still, if it would be a problem we can expand the navigation
metadata record with a new field that includes the UID for the record
where the data, if any, can be found for the offset fields that are
set to 0xffff, i.e. it would still be possible to include unique data
in a record even if we re-use data for some of the fields.

I've sent you a Plucker document that includes some simple navigation
data in a separate message.
