Depending on the license, it may be allowed to change logo and/or name
of the product..
Personally, I feel that modifying a logo (or any entity that represent
the product) is morally incorrect- even if allowed by the license.

Depends on the license again.

For GPL and other related licenses from FSF, check out the FSF
website. Or else, check out the opensource.org website for various
open source licenses.
