3128 is the default Squid port.  What do the ACL's on your Squid server
look like?  If it's wide open, it will definitely find its way into the
CBL, as open proxies can be very quickly subverted to send spam.  If
your machine forwards outbound connections to port 25 (as a NAT/MASQ
router for instance), and if there is any host behind it infected with
some kind of spam-generating malware (very common, if you have any
Windows boxes), that will very quickly put you on CBL.  This happened to
our GNU/Linux-based router once; I had to disable forwarding of port 25
(so now the only way to send mail would be to use the official mail
server) to get it delisted.
