I use MailScanner with spamassassin and clamav, and it works great. Advantages are low cost, easy maintenance, and it works well. I did have to upgrade my mailserver recently due to the spam volume, but depending on the server you can really put through a lot of mail in a day. And it's pretty easy to expand it out if you need more capacity by adding servers in parallel and using round-robin dns or another load-balancing method.
