Mark, My experience with hugin matches what Denis reported, so I'll take a stab at your questions.

I've always done this in hugin. My workflow is pretty simple with hugin -- I just follow the 'assistant' tab, and when it complains about control point auto-detection not working, it dumps me back at the 'control points' tab. Once here, just go through the images selecting control points manually. (Click a point on the left image, drag to fine-tune the location, then click close to the corresponding location on the right image and fine-tune further if necessary, then click 'add' in the lower right.) I usually set 5-8 control points per seam.
