Tom Limoncelli has a fine blog post on his fear of rebooting a server:

"Every time I reboot a machine I fear it won't come back up."

His solution set is simple, but not easy:

 1. Reboot, change, reboot. (The first reboot is interesting.)
 2. Reduce the number of services on a machine.
 3. Better testing.
 4. Better automation.

I score pretty well with #2 and #4, not so well on #1 and #3. (I tend to outsource #3 to the package managers at CentOS and Debian, which may not be a terribly trustworthy approach.)
