Likewise for me, I've had CenturyLink/Qwest for many years now.  A
standalone DSL line from them and Infinity Internet (iinet.com) for my
ISP.
I believe they're based in Vancouver (WA) but have a colo'd datacenter in
Portland.  Have had good luck with both with the (very few) times I've
actually
had to speak to a human.  Modem is a Dlink DSL-2640B which replaced
a crappy old Actiontec.  Have 1.5 Mb currently but can up that for only
$10 a month more -- that speed varies, of course, depending on location.
