Some weeks ago I reported on some odd effects I've been experiencing 
with Firefox on Ubuntu 12.04, GNOME Classic.

I have discovered a way to consistently duplicate the problem, and I 
think I have discovered a way to avoid the problem.

Open Firefox.
Open the history window (History > Show All History).
Minimize the history window.
Open the history window again, the same way as the first time. (History 
 > Show All History, as opposed to restoring the already opened history 
window.)

Using that approach, when the history window displays the second time, 
the title bar of the window is missing, along with the minimize, 
maximize, and close buttons. It is possible to get that title bar back 
by clicking on the window's button (called Library) on the task bar, 
which will minimize the window. Clicking on that button again restores 
the window and it has its title bar.

The solution I found was to start the Gnome Classic session with GNOME 
Classic (No effects). The missing title bar problem does not appear in 
the "No effects" mode. This mode also solves another problem where 
windows open with the title bar tucked up under the top bar on the 
desktop, the one with the menus, a bunch of icons for starting programs, 
notifications, time, etc. So, unless I discover something that I miss 
about the "effects," I'll stick with "no effects" from now on, until I 
figure out how to get things to work the way I wan them to in Ubuntu 
14.04. Maybe I'll even get that done before the next LTS release. :-)
