Your httpd.conf has an "Include /etc/httpd/extra/httpd-mpm.conf" statement, but in the mpm.conf all the MPM stanzas appear to be commented out and I don't see any "LoadModule" statement. e.g. "LoadModule mpm_worker_module modules/mod_mpm_worker.so"
