try checking these:

1. look in the logs for messages
2. check /etc/hosts.allow , /etc/hosts.deny, /etc/login.access 
3. check files /usr/sbin/tcpd , /usr/sbin/in.*
4. sometimes connection gets unpredictable if reverse dns maps to a
different hostname ... but sometimes it is ok (got to verify this)
5. you could migrate to xinetd, it is much more reliable.
