I get an occasional "duplicate" on a network that is having an
intermittent cabling problem - unreliable bnc connectors on thin ethernet.
I know that there are in fact NOT two machines with the same IP. Is it
also possible that dups are the result of ack's getting lost although the
original ping was, in fact, received?

Admittedly, this would also be a Bad Thing if it happened all the time.
I just ran across it yesterday when messing with my wiring. It happened
twice and hasn't happened again. I mention it because it shows that there
are other possible reasons for dups, albeit rare.
