In my experience, congested links lead to mis-communications between SMTP
servers, in which the sender aborts sending a message to a server it
mistakenly thinks that it timed out.  This in turn will force it to resend
that particular mail until the "roger" ack is received from the remote
SMTP server.  Even advanced MTAs like postfix announce this on 'mailq'
although it actually happens less on postfix than on sendmail.  Sendmail
really sucks in delivering email, and because of this the incidences of
duplicate email deliveries increase several-fold on sendmail.

My comment was basically on my experience managing smtp servers.
Filtering duplicate messages to the same client is a lot harder to do
especially in a busy email server.  It could be a good programming
exercise though for a delivery agent...  \8)
