it's a problem with the network address translator.  i may only be
configured to translate and allow tcp and udp packets but ignore and drop 
other IP packets like ICMP (which is used by ping).  when you
shutdown NAT, any packet passes through so now you can ping the apache
server (since it can also see your internal network) but cannot browse the
internet because request packets are no longer translated before exiting
your network.

or 

if your NAT is done in linux, you may have just forgotten to include
kernel support for ICMP Masquerading.
