I'm using the same card though I'm not sure with the chipset.
Try the following in your /etc/modules.conf (I used to use these
with V4L1):

alias char-major-89   i2c-dev
alias char-major-81   videodev
alias char-major-81-0 bttv
options i2c-algo-bit  bit_test=1
options bttv          card=34 radio=1
options tuner         type=2
