No, you don't have to.

You can create a Maildir in /etc/skel, the directory whose
contents are copied over by useradd onto a new system user's home
directory. Also place a .qmail there.

After this is in place, your user will automatically have a
Maildir and .qmail in his/her home directory whenever you create a
user by useradd.

You can pass in this option in your startup file using qmail-start.

For instance, you must have the following line in your
qmail startup file (In my server, it's /var/qmail/rc):

    qmail-start ./Maildir/

man qmail-start for more details.

If you have to do something differently because of IMAP, then I
may not be able to help you. Although I don't remember doing things
differently on a Corier-IMAP trial install I did before--long before.
