question number one, is the phenomenon reproducible? if so, there are 
ways of finding out where the error is. try reproducing the error on a 
relatively quiet time, and check the http access_log for how many POST 
operations did the web client actually make to send that email. if the 
send page is being called twice, then something must be wrong with the 
send page as rendered for that user. if there's only one POST entry, 
then check qmail logs for email sent from the apache web server. i 
assume, like any sane MTA, qmail logs all "local send email" 
invocations. if there are two invocations there, something must be wrong 
with the "submit message" as handled by the php app. if there's still 
one sendmail invocation, check the delivery for things like the 
destination is actually a forwarding address to a certain user twice. 
move forward until you reach the destination.

pero lam mo, medyo malabo na bug siya eh. kasi, for one, it only happens 
to one user. two, does it happen to all her recipients? on all her mail 
messages? kasi, i would believe that IMP is a very mature system for 
this kind of bug. look again more closely if you're up to it...component 
by component. good luck.
