I use:

Postfix + Cyrus IMAP + Mysql (via SASL/PAM/PAM-MySQL).

By virtual of the fact that there are more people who know SQL than LDAP,
using SQL is a much better approach for me.  I do however propagate the
email usernames to an LDAP directory purely for use of LDAP aware clients
needing a Global Address Directory.
