What USB devices are you using?  IIRC, you mentioned that this is from a
headless and keyboardless machine. Are you sure that your motherboard
chipset uses usb-ohci and not usb-uhci?  Also if you are not using any
USB devices then you should not load the USB modules. Comment out its
entry in your /etc/modules.conf. And disable hotplug using chkconfig.
