if you don't have /etc/tcp.smtp.cdb or if you dont have things like in an
uncompiled /etc/tcp.smtp:

127.:allow,RELAYCLIENT=""
:deny

if you are using plain tcpserver you should have -x /etc/tcp.smtp.cdb on
your command line, it specifies who is allowed to relay via your smtp
server.

the purpose of the /var/qmail/control/rcpthosts file specifies what domains
you will accept mail for, if the domain is not on this list, it will bounce
back the message.

well to simply test open relay, try doing the telnet xxx.xxx.xxx.xxx 25 mail
from, rcpt to dance, if you can send emails even if your ip is not allowed
as a relay client in the /etc/tcp.smtp.cdb then you are open relay, but i
doubt if you could send an email using the smtp server even if your ip range
is not allowed.
