Although it's more of a full-blown mailing list manager than an "email blast" application, you may want to look into Mailman[1]. I've heard some horror stories about getting it up and running, but personally it's pretty straightforward especially on my Debian setup (I set up and administrate the free.net.ph mailing lists[2]). While I don't have any mailing list that does massive email transmissions, other big-time mailing lists are managed by Mailman. Notable in this area are Sourceforge.net, Python.org and Zope.org. You'll also want an MTA setup that can handle the email, of course.
